On 30 November 2015, the fishing vessel COCK-A-WIT LADY, with five people on board, reported a crew member had gone overboard 23 nautical miles SSW of Clark's Harbour, Nova Scotia. The crew recovered and then attempted to resuscitate the crew member, who was pronounced deceased after being airlifted to hospital.
On 16 June 2015, the fishing vessel CFV 130214 was reported overdue while fishing snow crab on the waters of Placentia Bay, Newfoundland and Labrador. The Joint Rescue Coordination Centre (JRCC) conducted a search of the area and, on 17 June 2015, recovered the 3 deceased crew members. The vessel was not found and is believed to have sunk.
On 09 March 2015, the fishing vessel Four Ladies 2003, with 3 people on board, reported a crew member overboard 15 nautical miles south of Cape Sable Island, Nova Scotia. The fishing vessel 3 P's and a PA recovered the person, and rescue specialists from the Canadian Coast Guard Cutter Spray went on board to provide assistance. The person was transferred to an ambulance at West Head wharf, Nova Scotia, but was pronounced deceased at hospital.
On 14 July 2014, the bulk carrier AMAKUSA ISLAND grounded south of Gull Rocks when the vessel departed Prince Rupert, BC. The vessel started taking on water. No pollution was reported. The TSB deployed a team of investigators.
On 12 June 2014, the crab fishing vessel "Five Star", with 2 persons on board, was reported sunk, near Kelsey Bay in the Johnstone Strait. The master is missing and presumed drowned. The deckhand, wearing a personal floatation device, managed to swim ashore and contact 911. A TSB investigator was assigned to the accident.
On 11 February 2014, the tug Jose Narvaez, while towing an empty barge down the South Arm Fraser River, British Columbia, sustained a main engine crankcase explosion. The tug and barge were towed back to the dock and secured. No injury or pollution was reported.
On 25 January 2014, the container vessel "Cap Blanche" was inbound from Sand Heads, BC for Fraser Surrey Docks when the vessel grounded near Steveston, BC. The vessel was able to refloat itself without the assistance of tugs and proceeded to the berth. No pollution or injuries were reported.
On 14 October 2014, the tanker "NANNY" touched bottom south of Deer Island, Chesterfield Inlet, Nunavut. The tanker was outbound from Baker Lake, destined for the coast of Labrador, Nfld and Labrador, with a partial load of diesel on board. A 2 foot crack was reported in number 3 ballast tank allowing ingress of water. There were no injuries or pollution reported. The vessel continued on its voyage.
Tug Vachon and bulk carrier Orient Crusader

On 12 September 2014, the harbor tug "Vachon", while manoeuvring to berth the bulk carrier "Orient Crusader", made contact with the breakwater in Port Cartier, QC. Damage was reported to the tug and breakwater.
