On 11 July 2016, the tug Ken Mackenzie, with two people on board, reported a fire in the engine room while transiting the Fraser River, British Columbia. The two crew members abandoned the vessel onto the tug Harken No. 5. The fire was extinguished with the assistance of vessels in the vicinity. No injuries or pollution were reported. The tug sustained damage.
On 24 May 2016, the tug Albern, with 2 people on board, reported being struck by the tug C.T. Titan. The tug Albern subsequently sank in Northumberland Channel, British Columbia. The tug C.T. Titan recovered the 2 crewmembers of the tug Albern from the water. Approximately 800 litres of diesel fuel and 200 litres of hydraulic oil were reported to be on board of the tug Albern.
On 02 March 2016, the tug H.M. Scout lost its tow when the tow line parted 0.4 nautical miles south of Victoria, British Columbia. The barges H.M. Tacoma and H.M. Blue Horizon, subsequently went aground on the south coast of Vancouver Island, British Columbia. The broken tow line fouled the tug’s propeller and the tug returned to Victoria at slow speed. There were no reported injuries or pollution.

On 01 April 2016, the tug Ocean Uannaq, with two people on board, capsized and sank near the new Champlain Bridge on the St. Lawrence River off Montreal, Quebec. The tug was working with two other tugs to move a construction barge toward the shore. Both crew members were rescued. There were no injuries or pollution reported. The tug sustained major damage.
On 21 February 2016, the fishing vessel Saputi struck ice in the Davis Strait area and reported undetermined water ingress in the cargo hold. The heavily listed vessel proceeded to Nuuk, Greenland, under its own power with a Danish naval vessel standing by. No pollution or injuries were reported.
On 17 February 2016, the fishing vessel Bessie E, with 5 people on board, experienced a propulsion failure, while returning to Mamainse Harbour, Lake Superior, Ontario. The 5 crew members jumped to shore before the vessel capsized. There were no reported injuries. The vessel was a total constructive loss.
On 22 January 2016, the container vessel MSC MONICA ran aground 1 nautical mile NNE of Deschaillons-sur-Saint-Laurent, Quebec. The vessel was refloated the following day, with the assistance of three tugs, and escorted to Québec City, Quebec, to undergo the necessary inspections. No injuries or pollution were reported but the vessel sustained minor damage to the hull and the propeller sustained major damage.
On 06 September 2016, the fishing vessel Pop’s Pride, with four people on board, was reported overdue after it did not return to St. John’s, Newfoundland and Labrador, from the fishing grounds off Cape Spear, Newfoundland and Labrador. Several vessels participated in a search where two people were recovered deceased. The submerged vessel was recovered the following day. Two people were not recovered and are presumed drowned.
On , the passenger vessel Grandeur of the Seas, with approximately 3000 people on board, reported a close-quarters situation in thick fog with the passenger vessel Summer Bay, with 39 people on board, in the approaches to Halifax Harbour, Nova Scotia.
