Air transportation safety investigation A23C0048

The occurrence

On , a Bell Helicopter 206L-1 operated by Custom Helicopters Ltd. departed from Truelove Inlet, Nunavut, for Devon Island, Nunavut, with one pilot and two passengers on board. During the flight, the pilot lost visual reference and collided with the ice- and snow-covered terrain.

The passengers and pilot sustained minor injuries and were taken to the hospital. The helicopter was destroyed.

The occurrence

On 19 July 2023, the Valhalla Helicopters Inc. Bell Textron Inc. 205A-1 helicopter (registration C-GRUV, serial number 30198) was conducting operations in support of firefighting efforts in north-central Alberta. At 1803 Mountain Daylight Time, the helicopter departed Haig Lake firebase, Alberta, on a visual flight rules flight to a forest fire located approximately 15 nautical miles northeast of Peace River Aerodrome (CYPE), Alberta, with 1 pilot onboard and an empty bucket suspended below the helicopter on a 150-foot longline.

The occurrence

On 12 September 2023, a Cessna 208B operated by Superior Airways was conducting a flight from Red Lake Airport, Ontario with a pilot and 6 passengers on board.

On approach to Runway 27 at the Pikangikum Airport, Ontario, the aircraft entered fog and made a long landing. After touching down, the Cessna rolled off the end of the runway. The aircraft received damage to its propeller. There were no injuries.

The occurrence

On , a Synergy Aviation Ltd. Robinson R44II helicopter was conducting external load work 30 nautical miles east of Grand Cache, Alberta.

While the helicopter was lifting off from an oil field site, the longline that was connected to the cargo hook of the helicopter snagged a railing associated with the oil field infrastructure. The helicopter subsequently collided with terrain resulting in substantial damage.

The occurrence

On 04 November 2023, the Beech King Air A100 aircraft (registration C-GJJF, serial number B-123), operated by Propair Inc., was conducting a medical evacuation flight under instrument flight rules, from Rouyn-Noranda Airport (CYUY), Quebec, to Wemindji Airport (CYNC), Quebec, with 2 pilots and 3 mission personnel on board. At 0227 Eastern Daylight Time, the aircraft touched down slightly left of the centreline of Runway 28 at CYNC.

The occurrence

On 06 October 2023, the SkyQuest Aviation Ltd. Piper PA-34-200 Seneca aircraft departed Langley Regional Airport, British Columbia (BC), on a training flight with 1 instructor, 1 student and 1 passenger on board.

After conducting some manoeuvring exercises and a circuit at Chilliwack Airport, BC, the aircraft initiated a missed approach exercise. During the manoeuvre, the airplane rolled to the right and continued to roll until it struck trees and terrain in a nose-down and inverted attitude.

The occurrence

Shortly before 1100 Mountain Daylight Time on 06 October 2023, 2 aircraft tow vehicles (tugs) (call sign ATSTOW504 +1) operated by Airport Terminal Services at Calgary International Airport (CYYC), Alberta, planned to drive from Apron 1, to the south side of the airport to reposition an aircraft that was parked on Runway 26. The lead tug driver was given instructions by the ground controller to proceed south on taxiways C, C1, and Y, and hold short of Runway 26.

The occurrence

On 24 October 2023, a Sikorsky S-76C++ helicopter operated by Helijet International Inc. was conducting a flight from Vancouver Harbour Heliport, British Columbia (BC), to Victoria Harbour (Camel Point) Heliport, BC, with 2 flight crew members and 12 passengers on board. During cruise flight, the helicopter was struck by lightning and entered a rapid descent. The captain re-established straight and level flight after losing 3144 vertical feet. The flight crew continued the flight to Victoria and landed without incident.

At 1525 Eastern Daylight Time on 26 October 2023, the Boeing of Canada Ltd. De Havilland Division DHC-8-106 aircraft (registration C-FLSX, serial number 285) operated by Air Creebec Inc. began an instrument flight rules flight from Chisasibi Airport (CSU2), Quebec, to Val-d’Or Airport (CYVO), Quebec, with 2 flight crew members, 1 cabin crew member, and 28 passengers on board.
