On at approximately 1130 Pacific Daylight Time, the Royal Canadian Marine Search and Rescue vessel Lewis-McPhee, with 4 crew members on board, capsized during exercises in Sechelt Rapids, British Columbia. Two of the crew members were recovered by another vessel; the remaining 2 crew members were later found dead under the capsized vessel
On 30 May 2012, at 1000 Newfoundland Daylight Time, the passenger ferry Beaumont Hamel experienced an electrical failure, resulting in loss of propulsion and steering while approaching Portugal Cove, Newfoundland and Labrador, and struck the wharf. The vessel sustained damage to the bow visor, and caused minor damage to the wharf. One minor injury was reported.
On , the tanker Nanny ran aground on a shoal while outbound from Baker Lake, Nunavut, damaging the forward section of the hull. On 27 October, the vessel came off the shoal during strong northwesterly winds and later proceeded to St. John’s, Newfoundland and Labrador, for repairs. There were no injuries or pollution reported.
On , at approximately 0430 Pacific Daylight Time, the Canadian fishing vessel Viking Storm collided with the American fishing vessel Maverick in thick fog, 30 nautical miles off La Push, Washington. The Maverick capsized and sank from the impact; 3 of the 4 crew members on board survived and were rescued by the Viking Storm. The fourth crew member was never found and is presumed drowned.
On , at approximately 1340 Eastern Daylight Time, the passenger vessel Jiimaan grounded while approaching the ferry terminal in Kingsville, Ontario. The 34 passengers and crew remained on board until the vessel was refloated at approximately 1308 the next day, after which time the Jiimaan was escorted to the port of Leamington, Ontario. There was no pollution, damage, or injuries.

On at approximately 0415 Atlantic Daylight Time, the 8.69 m fishing vessel Ocean Star II, with 3 people on board, departed the wharf located near the community of Port Hood, Nova Scotia, to haul lobster traps. The wind was westerly at 10–15 knots and there was a 2 m northwest swell.
On , the bulk carrier Maccoa was forced to undertake evasive maneuvers to avoid a collision with a Canadian Pacific Railway bridge near Kahnawake, Quebec, due to the failure of the bridge span to rise. A presumed bottom contact was reported while the vessel was maneuvering.
