TSB Quarterly Review — Fourth quarter 2017-18

Chair's message

For the TSB, 2018 started up in high gear. In fact, over the last three months, we held two news conferences, deployed investigators to 16 occurrence sites across the country, and started 17 new investigations. We also released 23 investigation reports, the highest number per quarter in recent years. We owe this performance to the dedication of all TSB staff working diligently to continuously improve the safety of Canada's transportation system.

One of the investigation reports (R16M0026) published in February includes a recommendation calling on Transport Canada, railway companies and road authorities across Canada to take steps to improve safety at railway crossings that are designated for persons using assistive devices, such as wheelchairs, arm supports, hearing aids, visual aids, and others. Another high-profile investigation report (A16A0032) reminded us once again of the risks involved with continuing an unstable approach to a landing and highlighted the reasons why this issue is still on our Watchlist.

In the next few weeks, we will be releasing a new Occurrence Classification Policy as part of our ongoing modernization efforts. So stay tuned for more information. We are also working to identify the key safety issues that will comprise the next Watchlist edition, which will be published in late fall.
